青衫Aspie,神经科学博士在读的孤独症谱系人士 谢邀。人们往往知道 as 和精神变态(一般是反社会人格障碍)都存在共情问题,并认为 as 一样没有感情,不会同情。其实这是完全错误的观点,因为两者共情问题的方面是完全不同的。 首先简单科普,共情(同理心)分为情感共情和认知共情。情感共情是指对他人情感的体会和回应,包括情绪感染和同情表现。认知共情则为对他人意愿意图的解读。我看过的很多科普往往把两种共情混为一谈或者不加区分,会造成很多的误解。 根据研究,as 的共情障碍为认知共情障碍,即很难理解他人的意图。as 的情感共情是完整的,具有体会他人情感和同情回应的能力,甚至感受本身可以远远强于 nt,尤其是对他人的痛苦感受。并且,在 asd(孤独症谱系障碍)人士中多见强情感共情在其他生物甚至物品上的泛化。但是很多 as 在对他人情感的回应上存在问题,属于社交障碍范畴。 还有约 50%的 as 在对他人和自己情感的感知方面存在障碍,但是如果被告知或者他人情感过于强烈和明显,是有正常的情感共情反应的。研究认为这种表现本身并非来自于 asd,而是来自于述情障碍,即对他人和自己情绪的体验、描述障碍,可以理解成对情感反应迟钝。约 50%的 asd 人士存在述情障碍。此外,述情障碍也可以出现在其他障碍和 nt 中。 反社会人格障碍恰恰相反,在认知共情上,即理解他人的意图完全没有障碍。反社会人格障碍的共情问题是情感共情问题,不能被他人的情绪所感染;也难以对他人产生同情的情感。这就使得他们对他人要做什么理解非常清晰,但是对他人没有同情,甚至觉得他人的痛苦好笑。监狱中约有 50%的人有反社会人格障碍。 值得一提的是,认知共情概念和我们日常生活中的“情商”概念基本重叠(有“眼力健”)。反社会人格人士因为认知共情无障碍使得情商也会很高,并会利用他们的高情商操纵他人。 我的文章《为什么孤独症谱系存在共情问题》中对 asd 的共情问题有详细讨论。在此分享: 青衫 Aspie:为什么孤独症谱系存在共情问题 2010 年的一项研究检查了有无述情障碍的 asd 和脑共情相关信号。在 nt 中,观看他人接受造成疼痛的电击可以使左侧岛叶活动增加(与前人研究相符)。有述情障碍的 asd 人士这个信号有降低。但是,没有述情障碍的 asd 人士的信号和 nt 程度相当。但是,述情障碍的人也是在意他人的情感的,会被他人的愤怒等负面情绪产生不适感。 很多人认为,共情缺乏会导致反社会行为,其实这是不准确的。一项对有过违法暴力行为的人的研究显示,他们对他人受伤的痛苦情绪共情减少,但是认知共情正常。对反社会人格的研究也支持这一点。这种痛苦情绪共情减少、认知共情正常的模式,是与 asd 的共情问题完全不同的。 另外,受到新闻报道高智商犯罪的影响,人们往往认为反社会人格障碍都是高智商。对 as 的误解也是一样。在医学上这是完全不准确的。现阶段统计 as 约一半智力正常,一半超常。反社会人格的智力分布与常人无异。 也存在 as 和反社会人格障碍共同出现的情况。在一项对 50 名 as 成年男性的调查中,有 6 人(12%)存在反社会人格,但是样本量太小,不足以给出准确估计,而且反社会人格本来就是男性中更常见。没有其他与 as 或者 asd 与反社会人格的相关研究。 从另一个方面来看,一项瑞典的研究表明有孤独症诊断的人士犯罪率有升高 1.39 倍,但是调整其中的 adhd 和品行障碍情况后犯罪率与常人无异。研究人员认为这种表象的根源是 adhd 和品行障碍,并非 asd。 在人群中,反社会人格在男性中发生率在 4.5%-6.8%间,女性为 0.8-1.2%。在 adhd 人群中反社会人格发生率约为 12%(n = 5501)。asd 与 adhd 的共患率约为 15-35%。在上面提到的对 50 名 as 成年男性的研究中,adhd 出现率为 28%,落在上述对整个谱系的估算中。 对 asd 人士,我之前这篇共情学习提高策略共大家参考: 青衫 Aspie:学会共情:这些技巧让你变暖 参考文献: Mazza, Monica, et al.
